We don't add any crazy surcharges for shipping, just what it costs us to mail it -- postage, the cost of the mailer, labels, etc. There is also no charge for handling, just make sure Venus isn't looking!
USA
99% of the time we ship USPS. If you're getting one shirt, it's going out First Class. Two or three usually fit into a priority flat rate envelope. More than that and we experiment to see what package the order will fit into and what rates apply. The larger USPS flat rate boxes will fit 10-15 shirts, depending on the style and are still a pretty good deal. Anything in the 20+ shirt range will probably go UPS ground. Unless you specifically want a carrier, I'll go with what will be most economical and still get there in a reasonable amount of time.

Hash Mail
If you're in SoCal, or know someone who travels to the LA area regularly, we can probably get shirts to you via hash mail. It's a slow and barely reliable process, but we have seen it work. Once.
International Shipping
We've done a couple of international shipments and as long as we can use the USPS Priority flat rate mailers, it's not too bad. We'll take them on a case by case basis. Send us an email first as we only have US sales enabled on the web site. Flat rate envelopes are $11 to Canadialand and Mexico and $12 everywhere else. We haven't played around much with other options yet.
Returns
If we screwed up somehow (sent the wrong shirt, sent a misprinted shirt, wrong color, etc) we will send you the right shirt (hopefully) with a prepaid mailer for you to send the other shirt back.
We'll gladly exchange a shirt if you aren't totally happy with it (color, size, fit, whatever) as long as you haven't totally fouled it up. Please watch the deodorant and makeup and for pete's sake don't have sex with it please! Shipping's going to be on your dime both ways, so ask if you have sizing questions beforehand. We've gotten pretty good at looking at pictures and knowing what shirt will fit.
